ANNUAL MEDICAL FORM
Medical forms need to be completed upon enrollment and annually. Students are always growing and always changing, this includes their medical needs. It's important that MCC's Health Office be notified of any changes with a child's health. This information is kept in the strictest of confidences. The School Nurse will only share medical information with staff that are directly involved. Please complete the 2018-2019 Student Health Form and return to the school office.
Your children may qualify for free or reduced-price school meals. To apply, complete the Application for Educational Benefits following the instructions. At public schools, your application also helps the school qualify for education funds and discounts.
A new application must be submitted each year.

Open Enrollment approval/disapproval by the District is communicated to families by February 15 (or thirty days after application for late applications). After receiving approval of your application you must notify Murray County Central of your commitment to attend.
Transportation is only provided within the Murray County Central School District boundaries. Open enrolled students should contact Ludolph Bus Service at 507-836-6750 for transportation questions.
Keeping parents informed and involved helps to assure student safety and improve student success. With today's on-the-go lifestyles it has become more difficult for schools to reach families quickly and effectively. Murray County Central uses family information from the parent's JMC Parent Portal to load into the Alert System. Maintaining the accuracy of your contact information will increase the ability of the school to keep you informed.
